HSSE Pass and Permit

 

Vehicle and Machinery Permit Application

Vehicle permit comprises of three types of duration:
 
  1. Daily
  2. One (1) Week
  3. One (1) Month
Applicants, however, that can apply directly the permit only eligible to regular clients such as shipping agent, consignees and forwarding agent that frequently come to the port. The permit can be endorsed and approved at MCvT HSSE gatehouse.
 
For seasonal clients include companies, agencies and authorities that seldomly come to the port need to submit a letter application to MPC management.
 
The application is subject to the terms and conditions set out by HSSE Department in the General Port Safety Guideline and any other terms and conditions as may be implemented by MPC from time to time.
Instructions

To apply for the MPC vehicle and machinery permit, please complete the application form and submit it together with the following documents:

Vehicle:
  1. Personnel Name
  2. Identification Card No.
  3. Vehicle Licence Plate No.
 
Machinery:
  1. Operator’s and Signal Person’s Certificate
  2. Machinery Third Party Inspection Certificate
  3. Road Tax
 

All documents submitted shall be endorsed with the company’s stamp and submit to MCvT HSSE gatehouse to process the application. Once it is verified by the HSSE Supervisor, the applicant can proceed to MCT counter to pay for the toll fee and go back to MCvT HSSE gatehouse to issue the permit.

Permit-To-Work and Dangerous Goods (DG) Stuffing and Unstuffing Application

For the purpose of reviewing and approval of any high-risk activities in Muara Port, application of Permit-to-Work (i.e Hot Work, Lifting, High Altitude Operation, etc) and Dangerous Goods (DG) Stuffing and Unstuffing must be submitted via email to hseunit@muaraport.com .

Any required attachment and supporting documents must be attached inside the email as per required. The application can be made at least 48 hours in advance before the actual activity to commence.

Any individuals, companies or organization that intends to do a visit or enter Muara Port must submit an application to:

In this application letter, please do submit list of names, designation, organization, IC number, date, time and purpose of entry. The application must be made at least forty-eight (48) hours in advance before the actual entry.
